- 4AT Delirium Screening Tool: Explanation and Clinical Context
The 4AT Delirium Screening Tool is a rapid bedside assessment designed to identify delirium and cognitive impairment in hospitalized patients, particularly older adults and those with acute illness.
It consists of four domains including alertness, brief cognitive testing using AMT4, assessment of attention, and evaluation of acute onset or fluctuating course.
The tool does not require special training and can be completed within two minutes, making it suitable for routine clinical use in emergency departments, wards, and intensive care units.
A total score of zero suggests delirium is unlikely, a score between one and three indicates possible cognitive impairment, and a score of four or more is suggestive of delirium requiring further clinical evaluation.
The 4AT has been validated internationally and is recommended in several clinical guidelines due to its high sensitivity, practicality, and strong diagnostic performance in real world settings.
