Vasoactive-Inotropic Score

The Vasoactive-Inotropic Score (VIS) quantifies the intensity of cardiovascular pharmacological support using commonly used vasoactive and inotropic medications.

The score was developed in pediatric cardiac surgery and has subsequently been evaluated in multiple pediatric and adult critical care and cardiac surgery populations. Higher VIS values generally indicate a greater intensity of pharmacological cardiovascular support.

Standard VIS Formula
VIS = dopamine + dobutamine + (100 × epinephrine) + (100 × norepinephrine) + (10 × milrinone) + (10,000 × vasopressin)
Dopamine µg/kg/min × 1
Dobutamine µg/kg/min × 1
Epinephrine µg/kg/min × 100
Norepinephrine µg/kg/min × 100
Milrinone µg/kg/min × 10
Vasopressin U/kg/min × 10,000
Clinical note

VIS is a quantitative measure of pharmacological cardiovascular support. It should be interpreted in conjunction with the patient's clinical condition, hemodynamic status, treatment strategy, underlying disease, and the time point at which the score is measured.

The original Gaies study evaluated maximum VIS during the first 48 hours after pediatric cardiac surgery. Therefore, thresholds or prognostic associations reported in individual studies should not automatically be applied to every patient population or clinical setting.
