HOMA-IR (Insulin Resistance Index) Calculator
Calculate HOMA-IR
Enter fasting plasma glucose and fasting insulin measured from the same fasting assessment.
About HOMA-IR
The Homeostatic Model Assessment of Insulin Resistance (HOMA-IR) is a surrogate index estimating insulin resistance from fasting glucose and insulin concentrations. It is widely used in metabolic and cardiometabolic research, but it is not a direct measurement of insulin sensitivity.
Formula
HOMA-IR = fasting glucose (mg/dL) × fasting insulin (µU/mL) ÷ 405
When glucose is expressed in mmol/L, the corresponding conventional formula is fasting glucose × fasting insulin ÷ 22.5. This calculator accepts mg/dL only.
Clinical context
- Use fasting glucose and insulin values obtained under appropriate fasting conditions.
- Results can vary with population characteristics, age, sex, metabolic status, insulin assay, and study methods.
- HOMA-IR should not be used alone to diagnose diabetes or determine an individual's treatment.
Interpreting the result
Higher HOMA-IR values are generally consistent with greater insulin resistance, but a single universal diagnostic threshold has not been established.
The original tool mentioned values above approximately 2.0–2.5 as suggestive of insulin resistance. Because published cutoffs differ across populations and assays, this range is not presented here as a universal diagnostic rule.
For research, prespecify and cite the cutoff or reference distribution validated for the study population. For individual care, consider the full clinical picture and local laboratory context.
