INSPIRE Index (Insulin Sensitivity Prognosis) Calculator
Metabolic assessment
Estimate HOMA-IR from fasting plasma glucose and fasting insulin. Enter values from a fasting sample and confirm the units before calculating.
About HOMA-IR
Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) is a model-based estimate derived from fasting glucose and insulin concentrations. It is commonly used in clinical research and epidemiology; it is not a direct measurement of insulin sensitivity.
Calculation and units
- Glucose in mg/dL and insulin in µU/mL: multiply the values and divide by 405.
- Glucose in mmol/L and insulin in µU/mL: the corresponding conventional formula divides the product by 22.5. This calculator does not accept mmol/L input.
- Higher HOMA-IR values generally suggest greater insulin resistance, but interpretation depends on the population, assay, and clinical context.
Interpreting the result
Do not interpret this value using a universal low, moderate, or high-risk band. Published thresholds vary, and a cutoff should be selected only when supported for the relevant population and intended use. HOMA-IR alone does not diagnose diabetes or establish cardiovascular prognosis.
