  • Fontan Failure Index: Explanation and Clinical Context
The Fontan Failure Index aims to integrate hemodynamic burden (e.g. elevated central venous pressure, reduced cardiac index) and hepatic injury (represented by liver stiffness by elastography and thrombocytopenia as marker of portal hypertension) to stratify risk of clinical Fontan failure. A higher score suggests greater circulatory stress and end‑organ (hepatic) compromise, which may precede overt clinical decompensation. Clinically, this tool can support longitudinal surveillance and prompt earlier referral for advanced therapies (e.g. Fontan revision, transplant evaluation).

    Reference:
Kramer P, et al. A Multimodal Score Accurately Classifies Fontan Failure and Late Mortality in Adult Fontan Patients. (PMC) Additional supporting context: in Fontan patients, severity of liver disease correlates with hemodynamics (e.g. elevated filling pressures).
