  • CARPREG II Score (2018 Updated Version): Explanation and Clinical Context
The CARPREG II risk score is an evidence-based tool to estimate the risk of maternal cardiac complications during pregnancy for women with structural heart disease.
It uses clinical variables including prior cardiac events or arrhythmia, baseline NYHA class > II or cyanosis, mechanical heart valve, reduced systemic ventricular function (EF <40%), left heart obstruction, and pulmonary hypertension.
Each variable is weighted according to its contribution to risk based on multivariate analysis in the original CARPREG II 2018 cohort.
The total score stratifies patients into low, moderate, and high risk for maternal cardiac events, providing a quantitative tool for risk counseling, monitoring intensity, and management planning.
