  • Diabetes requiring medication
  • Second prior myocardial infarction (≥1 previous MI in addition to index event)
  • Multivessel coronary artery disease (documented)
  • Chronic non–end-stage renal dysfunction — Creatinine clearance (mL/min, Cockcroft–Gault) Enter estimated creatinine clearance (e.g., Cockcroft–Gault). PEGASUS criterion met if CrCl < 60 mL/min.
  • History of major bleeding, oral anticoagulant therapy, or recent GI bleed? (for clinical exclusion)
