  • POSSUM and P POSSUM Mortality Score Explanation and Clinical Context
POSSUM Physiological and Operative Severity Score for the Enumeration of Mortality and Morbidity is a validated model used to predict postoperative mortality by combining physiological and operative parameters. The score integrates age cardiovascular respiratory status hemodynamic variables hematologic markers electrolyte balance and operative burden to produce a mortality estimate. It is widely used for quality assessment surgical risk stratification and benchmarking of surgical outcomes. P POSSUM Portsmouth modification of POSSUM was developed to improve calibration for mortality prediction particularly for lower risk groups where the original POSSUM tended to overpredict. P POSSUM adjusts the regression coefficients and provides a more accurate mortality estimate across diverse surgical populations. Both scores allow clinicians to quantify perioperative risk align perioperative planning improve patient counseling and support shared decision making. They also serve as tools for auditing surgical performance and evaluating institutional outcomes.
