SCORE2-Diabetes

1. Patient characteristics

ESC recommended calculation population: 40 years and older; this calculator limits input to 40–69 years.
Select only when applying the corresponding European calibration. Indonesia is not assigned to an ESC SCORE2 risk region.

2. Cardiovascular history and target-organ damage

Includes severe kidney disease/proteinuria or microvascular disease in at least three sites (see definition below).
Patients with established ASCVD or severe TOD are classified as very high cardiovascular risk by ESC criteria; SCORE2-Diabetes risk calculation is not required for this classification.

3. Conventional cardiovascular risk factors

4. Diabetes-specific variables

The model uses IFCC mmol/mol. NGSP % is converted internally.

About SCORE2-Diabetes

SCORE2-Diabetes estimates the 10-year risk of fatal and non-fatal cardiovascular disease (cardiovascular death, non-fatal myocardial infarction, or non-fatal stroke) in people with type 2 diabetes without previous cardiovascular disease.

The model extends SCORE2 by incorporating diabetes-specific predictors and competing-risk-adjusted, sex-specific equations. Its original calibration is specific to four European cardiovascular risk regions.

Model variables

DomainVariables
DemographicAge and sex
Conventional riskCurrent smoking, systolic blood pressure, total cholesterol, HDL cholesterol
Diabetes-specificAge at diabetes diagnosis, HbA1c, eGFR

ESC risk categories

Category10-year CVD risk
Low<5%
Moderate5% to <10%
High10% to <20%
Very high≥20%, or established ASCVD/severe TOD

Severe target-organ damage (ESC 2023)

  • eGFR <45 mL/min/1.73 m², irrespective of albuminuria; or
  • eGFR 45–59 mL/min/1.73 m² with UACR 30–300 mg/g; or
  • UACR >300 mg/g; or
  • Microvascular disease in at least three different sites.

Clinical disclaimer: This calculator is an educational clinical decision-support aid and does not replace clinical assessment or guideline-based decision-making. SCORE2-Diabetes was developed and calibrated for European populations. The four calibration regions are not validated substitutes for Indonesia or other non-European populations. Do not interpret a regional estimate as a locally calibrated absolute risk. Patients with established ASCVD or severe target-organ damage should be classified using the relevant clinical criteria rather than relying on this calculation.
