About the NCDR CathPCI risk models

This page presents three distinct published bedside risk scores for patients undergoing percutaneous coronary intervention (PCI). They estimate different outcomes and should not be interpreted as a single composite score.

Model scope and outcome definitions

  • In-hospital mortality: the 2021 CathPCI Registry model was developed using 706,263 PCIs from July 2018 to June 2019. The simplified bedside score uses age, CKD stage, clinical instability and cardiac-arrest responsiveness.
  • Major bleeding: the 2013 NCDR score predicts major bleeding within 72 hours after PCI. The registry definition includes a hemoglobin decrease of at least 3 g/dL, transfusion, or intervention to stop/correct bleeding.
  • AKI: the updated 2023 bedside model predicts post-PCI AKI. In the study, AKI was defined as a serum creatinine increase of ≥0.3 mg/dL or ≥50% from baseline. The derivation used 2020 PCI data and excluded patients already receiving dialysis or without post-procedure creatinine data.

Interpretation and limitations

These bedside point scores are simplified models, not the full NCDR risk-adjustment equations. The mortality model's full risk model and the registry's institutional benchmarking software are distinct from this bedside score. The bleeding score is based on an older registry era; the AKI score is a more contemporary update. Calibration may differ across countries, hospitals, procedural practices and patient populations.

Clinical instability categories must be classified using the source registry definitions. This calculator does not replace formal NCDR data definitions, clinical judgement, or local validation.
