ASSIGN Cardiovascular Risk Score Calculator
10-Year Cardiovascular Risk Assessment
Enter the patient's risk factors to estimate 10-year ASCVD risk using the recalibrated ASSIGN v2.0 equation.
This estimate is intended for primary prevention in the population for which ASSIGN v2.0 was developed. It is not validated for direct application to other countries or ethnic populations.
About the ASSIGN Score
ASSIGN (ASsessing cardiovascular risk using SIGN guidelines to assign preventive treatment) estimates the 10-year risk of a first atherosclerotic cardiovascular disease (ASCVD) event. The recalibrated ASSIGN v2.0 was developed for people living in Scotland without established cardiovascular disease.
The model includes conventional cardiovascular risk factors together with family history and socioeconomic deprivation measured using the Scottish Index of Multiple Deprivation (SIMD).
Variables included in ASSIGN v2.0
| Variable | Model input |
|---|---|
| Biological sex | Male or female; sex-specific equation |
| Age | Years (continuous) |
| Total cholesterol | mmol/L (continuous) |
| HDL cholesterol | mmol/L (continuous) |
| Systolic blood pressure | mmHg (continuous) |
| Diabetes mellitus | Yes / no |
| Family history of CVD | Mother, father, or sibling |
| Cigarette consumption | Number per day |
| Socioeconomic deprivation | SIMD score / 10 |
Interpretation
The calculated value is the estimated percentage probability of a first ASCVD event within 10 years. Under the Scottish ASSIGN v2.0 pathway, a risk of 10% or greater is considered high risk for prevention assessment. This is not a universal treatment threshold and should not be transferred to other national guidelines without validation.
- Below 10%: Below the Scottish v2.0 high-risk threshold.
- 10% or greater: Meets the Scottish v2.0 high-risk threshold.
People with established CVD, familial hypercholesterolaemia, or specified high-risk diabetes/CKD categories may not require risk scoring; follow the applicable guideline pathway instead.
