HOSPITAL Score Calculator – 30-Day Unplanned Hospital Readmission Risk
30-Day Readmission Risk Assessment
Enter the patient's discharge and admission characteristics.
The displayed risk percentages are observed rates in the international validation cohort, not individualized or universally calibrated probabilities.
About the HOSPITAL Score
The HOSPITAL score is a clinical prediction model designed to identify adult medical patients at risk of potentially avoidable 30-day readmission after hospital discharge. The acronym represents Hemoglobin, Oncology, Sodium, Procedure, Index admission type, number of prior Admissions, and Length of stay.
The score was derived and subsequently validated in hospitalized medical patients. Its use should remain aligned with the population and outcome definitions of the original studies.
Variables and scoring
| Variable | Criterion | Points |
|---|---|---|
| Hemoglobin | <12 g/dL at discharge | 1 |
| Oncology | Discharge from oncology service | 2 |
| Sodium | <135 mmol/L at discharge | 1 |
| Procedure | Procedure during index stay | 1 |
| Index admission | Urgent / emergency | 1 |
| Prior admissions | 0–1 / 2–5 / >5 in previous 12 months | 0 / 2 / 5 |
| Length of stay | ≥5 days | 2 |
Risk groups
| Total score | Risk group | Observed readmission rate |
|---|---|---|
| 0–4 | Low | 5.8% |
| 5–6 | Intermediate | 11.9% |
| ≥7 | High | 22.8% |
Important methodological note
The international validation publication used prior admissions categories of 0–1, 2–5, and more than 5. This calculator uses that validated categorization. The derivation and validation publications should be consulted for details of cohort selection, outcome definition, and model performance.

Scope and limitations
- Designed for adult medical patients, not as a universal score for all inpatient populations.
- Predicts potentially avoidable readmission as defined in the source studies; this is not identical to every unplanned readmission.
- Observed risk rates may differ across hospitals, health systems, and patient populations.
- Use alongside clinical judgement and local readmission-prevention pathways.
