About the mNUTRIC Score
The modified Nutrition Risk in the Critically ill (mNUTRIC) score is an ICU-specific risk assessment tool developed to identify critically ill patients in whom nutrition risk is associated with adverse clinical outcomes. It uses five variables reflecting age, illness severity, comorbidity burden, and time spent in hospital before ICU admission.
The modified version omits interleukin-6 (IL-6) from the original NUTRIC score. The mNUTRIC total ranges from 0 to 9.
Variables and Points
| Variable | Value | Points |
|---|---|---|
| Age (years) | <50 / 50–74 / ≥75 | 0 / 1 / 2 |
| APACHE II | <15 / 15–19 / 20–27 / ≥28 | 0 / 1 / 2 / 3 |
| SOFA | <6 / 6–9 / ≥10 | 0 / 1 / 2 |
| Comorbidities | 0–1 / ≥2 | 0 / 1 |
| Hospital-to-ICU days | <1 / ≥1 | 0 / 1 |
Interpretation
| mNUTRIC score | Category |
|---|---|
| 0–4 | Low nutritional risk |
| 5–9 | High nutritional risk |
Clinical use
- A high score is associated with worse outcomes in critically ill populations; it is not a diagnosis of malnutrition.
- Use the score alongside a comprehensive nutrition assessment, clinical examination, history of weight and intake changes, and evaluation of muscle mass where feasible.
- The score does not independently prescribe calorie/protein targets or determine the route or timing of nutrition support.
