    This calculator helps evaluate likelihood of acute myocardial infarction based on serial high sensitivity troponin measurement using initial and repeat values combined with absolute and relative delta changes. Diagnosis is supported when at least one value exceeds the ninety ninth percentile reference limit with a rising or falling pattern that meets significant change threshold. A small delta change below normal limits favors non ischemic injury or low probability MI while marked increase suggests acute ischemia requiring ACS management pathway. Guidelines recommend hs Troponin testing at zero hour and repeated at one to two hours for rapid rule in or rule out. Absolute increase of five ng per liter or relative increase of twenty percent or more is generally considered significant depending on specific assay thresholds. When values fall into intermediate or inconclusive range clinicians should continue observation repeat sampling and integrate symptoms ECG and risk profile. Decision is never based on troponin alone but on entire ACS clinical context.
