EDACS (Emergency Department Assessment of Chest Pain Score)

Clinical calculation support · Verify results before clinical use

Estimate the Emergency Department Assessment of Chest Pain Score (EDACS) in adults presenting with possible acute coronary syndrome. The score is one component of the EDACS accelerated diagnostic pathway and must be interpreted with ECG and serial troponin results.

EDACS was developed for adult patients with possible cardiac chest pain.

Coronary artery disease and risk factors

In patients aged 18–50 years, add 4 points if the patient has known coronary artery disease or at least 3 listed cardiovascular risk factors.

Cardiovascular risk factors

0 risk factors selected

Risk factors: hypertension, dyslipidemia, diabetes, current smoking, or family history of premature CAD in a first-degree relative (age <55 years). The +4 component applies only at age 18–50; known CAD is prior MI, CABG, or PCI.

Pain radiating to arm or shoulder (+5 points)
Pain worsened with inspiration (−4 points)
Pain reproducible with palpation (−6 points)

EDACS-ADP assessment

Complete these fields to assess the pathway criteria in addition to the EDACS score.

Use the correct assay-specific reference limit and a locally validated serial troponin protocol. This simplified 0/2-hour assessment does not calculate assay-specific high-sensitivity troponin deltas and must not replace your institution's ACS pathway.

About the EDACS Score

The Emergency Department Assessment of Chest Pain Score (EDACS) is a clinical prediction score developed to risk-stratify adults presenting to the emergency department with possible acute coronary syndrome. It was derived using clinical predictors of 30-day major adverse cardiac events.

The EDACS score incorporates age, sex, coronary artery disease or cardiovascular risk factors, and selected chest-pain characteristics. The +4 coronary disease/risk-factor component applies only to patients aged 18–50 years. Do not add this component for patients older than 50 years.

EDACS-ADP low-risk criteria

  • EDACS score less than 16.
  • No new ischemia on the initial ECG.
  • Cardiac troponin concentrations are non-elevated at 0 and 2 hours, interpreted using the applicable assay and local protocol.
A score of 16 or higher does not by itself diagnose ACS. It means the patient does not meet the EDACS low-risk score threshold.

Clinical disclaimer: EDACS is a risk-stratification instrument and must not be used as a stand-alone diagnostic test or as the sole basis for discharge. A low EDACS score does not exclude acute coronary syndrome. Interpret the complete EDACS-ADP in the context of ECG findings, serial assay-specific troponin testing, symptom timing, clinical assessment, comorbidities, and local protocols. Patients with ongoing or recurrent symptoms, hemodynamic instability, ischemic ECG changes, elevated troponin, or high clinical suspicion require appropriate urgent evaluation regardless of score.
