    The HEART Score is a clinical risk stratification tool designed to evaluate patients presenting to the emergency department with chest pain. It focuses on identifying the risk of major adverse cardiac events within the next thirty days. The acronym HEART represents History, ECG, Age, Risk factors, and Troponin. Each component is scored from zero to two, producing a total score between zero and ten. The risk categories are divided into low, intermediate, and high, which correspond to increasing probabilities of adverse cardiac outcomes. Low risk patients typically have a very small likelihood of short term cardiac complications and may be suitable for discharge with outpatient follow up. Intermediate risk patients often benefit from prolonged observation and additional diagnostic workup such as advanced imaging or serial troponin measurements. High risk patients require urgent cardiology evaluation and aggressive management because of the significantly elevated risk of myocardial infarction or death. The HEART Score has been validated in multiple large prospective studies and is widely incorporated into emergency care pathways because of its simplicity, predictive accuracy, and ability to reduce unnecessary hospital admissions.
