- MELD-HF Score (Transplant Candidacy): Explanation and Clinical Context
The MELD-HF Score is a modified version of the original Model for End-stage Liver Disease (MELD) used to prioritize patients with advanced heart failure for transplant.
It incorporates key laboratory parameters: serum creatinine, total bilirubin, INR, and serum sodium, reflecting renal function, hepatic function, coagulation status, and electrolyte balance, which are critical in end-stage heart failure.
The score predicts mortality risk and helps clinicians determine transplant urgency and candidacy.
Higher MELD-HF scores indicate more severe multi-organ dysfunction and higher priority for heart or combined heart-liver transplantation.
