About the NAFLD Fibrosis Score
The NAFLD Fibrosis Score (NFS) is a non-invasive clinical score developed to estimate the likelihood of advanced fibrosis in adults with non-alcoholic fatty liver disease (NAFLD), using six routinely available variables.
Formula
NFS = −1.675 + (0.037 × age) + (0.094 × BMI) + (1.13 × IFG/diabetes) + (0.99 × AST/ALT ratio) − (0.013 × platelet count) − (0.66 × albumin).
Use age in years, BMI in kg/m², AST and ALT in U/L, platelet count in ×10⁹/L, and albumin in g/dL. Code impaired fasting glucose/diabetes as 1 when present and 0 when absent.
Cutoffs and interpretation
| NFS result | Interpretation | Clinical meaning |
|---|---|---|
| < −1.455 | Low | Helps rule out advanced fibrosis |
| −1.455 to 0.676 | Indeterminate | Cannot confidently rule out or rule in advanced fibrosis |
| > 0.676 | High | Supports a higher likelihood of advanced fibrosis |
Important limitations
- NFS estimates the likelihood of advanced fibrosis; it does not directly measure fibrosis or independently confirm cirrhosis.
- Interpretation depends on the population, prevalence of advanced fibrosis, comorbidities, and the accuracy and units of input values.
- An indeterminate or high result should prompt consideration of further assessment under an appropriate clinical pathway; a low result does not override strong clinical suspicion.
