    High sensitivity cardiac troponin is the preferred biomarker for evaluating suspected acute coronary syndrome. AHA 2025 guideline states that hs Troponin allows earlier rule out or rule in of myocardial infarction. When evaluating a patient who presents at least three hours from symptom onset a single hs Troponin below the assay specific ninety ninth percentile upper reference limit may support rule out particularly in low risk clinical settings. However values at or above the URL indicate myocardial injury and require serial sampling typically one to two hours later combined with ECG and clinical assessment. This tool aids rapid bedside interpretation using user entered assay cutoffs because each hospital may have different URL values. Clinical decisions should include symptoms stability ECG findings and risk scores when available.
