  • MAGGIC Mortality Risk Score: Explanation and Clinical Context
The MAGGIC (Meta-Analysis Global Group in Chronic Heart Failure) Risk Score is a validated prognostic model that estimates all-cause mortality in patients with heart failure, developed from data pooled from over 39,000 patients across 30 studies.
It incorporates simple, readily available clinical variables such as age, sex, NYHA class, LVEF, systolic blood pressure, weight, serum creatinine, and the presence of comorbidities including diabetes, COPD, and smoking. Medication use (ACE inhibitors or ARBs and beta-blockers) is also considered.
The score provides a continuous estimate of risk, which can be expressed as an estimated 1-year mortality percentage. This helps clinicians stratify risk, guide discussions about prognosis, and inform therapeutic intensity.
Higher MAGGIC scores indicate greater mortality risk, particularly in older patients with advanced NYHA class, low ejection fraction, renal dysfunction, and absence of guideline-directed medical therapy.
