    The BODE index is a multidimensional prognostic tool that combines body mass index BMI airflow obstruction measured as FEV1 percent predicted dyspnea assessed by the modified Medical Research Council mMRC scale and exercise capacity measured as six minute walk distance in meters. Each component is scored and the total ranges from zero to ten points. Higher scores are associated with worse survival and increased risk of hospitalization. Typical scoring thresholds are for FEV1 percent predicted of greater than or equal to 65 percent zero points 50 to 64 percent one point 36 to 49 percent two points and 35 percent or less three points. For six minute walk distance the thresholds are greater than or equal to 350 meters zero points 250 to 349 meters one point 150 to 249 meters two points and less than 150 meters three points. For dyspnea mMRC grades zero to one receive zero points grade two one point grade three two points and grade four three points. For body mass index a value of 21 kg per m2 or less scores one point otherwise zero points. The total BODE score categorizes prognosis with lower scores indicating higher estimated four year survival and higher scores indicating lower estimated four year survival. Clinical use includes risk stratification guidance for referral to pulmonary rehabilitation consideration of advanced therapies and communication with patients and families about prognosis.
