    This index is a proposed composite metric designed to reflect the synergistic effect of diabetes mellitus and cardiovascular disease (CVD) risk in patients. Key input variables include age, gender, duration of diabetes, glycaemic control (HbA1c), prior cardiovascular history and estimated glomerular filtration rate (eGFR). The underlying rationale is that diabetes and cardiovascular disease share pathophysiologic mechanisms—such as endothelial dysfunction, insulin resistance, dyslipidaemia, hypertension—and that their interaction amplifies risk rather than simply adding linearly. Clinical significance: Patients with long-standing diabetes, suboptimal HbA1c, prior cardiovascular events and declining renal function are at markedly increased cardiovascular risk. Incorporating a “diabetes-CVD interaction” metric may help stratify diabetic patients with heightened cardiovascular risk and prompt more intensive risk factor management. Interpretation summary: Although no formal cut-offs exist yet, clinicians may consider higher values (for example >5) as indicating elevated risk and warranting further cardiovascular assessment and aggressive risk-factor control (glycaemic, lipid, blood pressure, lifestyle). This tool is not validated and should complement, not replace, established cardiovascular risk scores.
