Hestia Criteria (Outpatient PE Eligibility) Calculator

  • Hemodynamic Stability (SBP >100 mmHg, no shock)
  • Active bleeding or high risk of bleeding
  • Need for oxygen to maintain saturation >90%
  • Severe comorbidity requiring hospital admission (e.g., heart failure, renal failure, severe liver disease)
  • Other medical or social reason requiring hospital admission (e.g., poor home support, compliance issues)
  • Hestia Criteria (Outpatient Pulmonary Embolism Treatment Eligibility): Explanation and Clinical Context
The Hestia criteria provide a validated, simple tool to identify patients with acute pulmonary embolism who may be safely treated as outpatients.
The criteria include hemodynamic stability, absence of active or high-risk bleeding, no need for supplemental oxygen to maintain saturation above 90%, no severe comorbidities requiring hospital admission, and no social or medical barriers to outpatient care.
Patients who meet all criteria (i.e., zero flags) are considered suitable for outpatient management, which has been shown to be safe and effective, reducing hospital stay and healthcare costs without compromising clinical outcomes.
