Quick SOFA (qSOFA) Calculator

  • Respiratory Rate > 22 breaths/min
  • Systolic Blood Pressure < 100 mmHg
  • Altered Mentation (GCS < 15)
  • Quick SOFA (qSOFA) Score: Explanation and Clinical Context
The Quick Sequential Organ Failure Assessment (qSOFA) score was proposed by the Sepsis-3 task force (Singer et al., 2016) as a simplified bedside tool to identify patients with suspected infection at greater risk for poor outcomes outside the intensive care unit.

It uses three simple clinical criteria: respiratory rate > 22 breaths per minute, systolic blood pressure < 100 mmHg, and altered mental status (Glasgow Coma Scale < 15). Each criterion scores one point, for a total of 0–3.

A qSOFA score of >2 suggests a higher likelihood of sepsis with increased risk of mortality and prolonged ICU stay, warranting prompt evaluation for organ dysfunction using the full SOFA score. However, qSOFA is not a diagnostic tool for sepsis, but rather a rapid bedside screening tool for risk stratification.

Clinicians should interpret qSOFA in the context of the patient's overall presentation, laboratory data, and infection source. Its simplicity and applicability outside the ICU make it valuable in emergency and ward settings, particularly where laboratory results are not immediately available.
