Serial measurement of high sensitivity cardiac troponin plays a pivotal role in the diagnosis of acute coronary syndrome. The 2025 AHA ACS guideline emphasizes rapid rule in and rule out assessment using baseline and 1 to 2 hour repeat hs troponin values combined with clinical history and ECG findings. Serial trajectory interpretation is more accurate than relying on a single value because myocardial necrosis evolves over time. A rising pattern with significant delta elevation strongly correlates with acute myocardial infarction particularly when values exceed the ninety ninth percentile threshold. A small or flat trajectory supports low risk status especially when both values remain below reference upper limit normal. Intermediate delta patterns require continued observation and repeat testing since myocardial injury may be evolving. Troponin elevation alone does not confirm type one MI and must be correlated with ischemic symptoms ECG changes and clinical risk profile.
