SOFA Score for Organ Dysfunction Explanation and Clinical Context
The Sequential Organ Failure Assessment score is a clinical tool that evaluates the severity of organ dysfunction across six major organ systems. These systems include respiration coagulation liver function cardiovascular status central nervous system and renal function. Each system receives a subscore between zero and four based on objective clinical and laboratory values. The total score reflects the global burden of organ dysfunction. Higher scores indicate greater severity of illness and are associated with increased mortality risk. The score is widely used in intensive care units to monitor progression or improvement over time and to support prognostic discussion. It is also used in sepsis diagnosis where an increase in score of two or more points represents a significant change in clinical status.

The respiratory component assesses the ratio of arterial oxygen to inspired oxygen and the need for invasive ventilation. The cardiovascular component considers mean arterial pressure and vasopressor support. Renal assessment can be based on serum creatinine or urine output. Consistent measurement and trending over time provide valuable insight into a patient clinical trajectory.
