  • MADIT-CRT Response Score: Explanation and Clinical Context
The MADIT-CRT Response Score is a clinical tool developed to predict the likelihood of a favorable response to cardiac resynchronization therapy with defibrillator (CRT-D) in patients with heart failure and mild symptoms. The score is based on seven baseline clinical and echocardiographic variables: female sex, non-ischemic etiology, left bundle branch block (LBBB), QRS duration >150 ms, prior hospitalization for heart failure, left ventricular end-diastolic volume >125 mL/m², and left atrial volume <40 mL/m².

A higher score indicates a greater likelihood of a positive response to CRT-D, which is associated with improved survival and reduced hospitalization for heart failure. Conversely, a lower score suggests a reduced benefit from CRT-D, highlighting the importance of patient selection in CRT therapy.
