- SCORTEN Severity of Illness Score for Toxic Epidermal Necrolysis
SCORTEN is a validated prognostic index intended to estimate the probability of death in patients with Stevens Johnson syndrome and toxic epidermal necrolysis when assessed within the first 24 hours of admission. The score comprises seven independent variables each scored as one point if present. These variables are age 40 years or older, presence of active malignancy, heart rate 120 beats per minute or greater, percentage of body surface with epidermal detachment at admission 10 percent or greater, serum urea greater than 28 mg per dL, serum bicarbonate less than 20 mEq per L, and serum glucose greater than 250 mg per dL. The total score correlates with increasing mortality risk for example total score 0 to 1 corresponds to about 3.2 percent mortality and score 2 corresponds to about 12.1 percent mortality and score 3 corresponds to about 35.3 percent mortality and score 4 corresponds to about 58.3 percent mortality and score 5 or more corresponds to greater than 90 percent mortality.
