  • EPIPHANY Index (Pulmonary Embolism in Cancer): Explanation and Clinical Context
The EPIPHANY Index is a prognostic tool developed for patients with active cancer who present with pulmonary embolism. It estimates the risk of 30-day serious complications, including mortality, hemodynamic instability, or need for intensive care interventions.
The score integrates clinical parameters available at presentation: oxygen saturation, systolic blood pressure, heart rate, prior history of venous thromboembolism, ECOG performance status, unilateral leg swelling, and dyspnea.
Each variable is assigned points based on its association with adverse outcomes. The cumulative score stratifies patients into low, intermediate, or high risk.
This tool aids oncologists and pulmonologists in risk stratification, guiding decisions regarding outpatient management, anticoagulation intensity, and monitoring needs.
