Kocher Criteria were developed to help clinicians estimate the likelihood of septic arthritis in children presenting with acute hip pain. The system uses four clinical predictors which are history of fever inability to bear weight erythrocyte sedimentation rate above forty mm per hour and serum white blood cell count above twelve thousand per microliter. Each positive predictor contributes one point to the total score. Higher scores correlate with a progressively greater probability of true septic arthritis.
These criteria guide decisions regarding urgent joint aspiration initiation of empiric antibiotics and possible surgical drainage. While Kocher Criteria remain widely used clinicians should interpret them in conjunction with clinical examination and imaging findings because performance can vary across populations.
