  • Target Cardiac Output (CO): Explanation and Clinical Context
Target cardiac output represents the estimated cardiac output required to maintain adequate systemic perfusion in relation to an individual's body surface area. The formula uses a standard cardiac index value of 2.2 liters per minute per square meter, which reflects the minimal cardiac index associated with preserved organ perfusion in hemodynamically stable adults. Cardiac output is a central indicator of the heart's ability to deliver oxygenated blood to tissues, and it is influenced by preload, afterload, myocardial contractility, and heart rate. Normal values for cardiac output in adults generally range from 4.0 to 8.0 liters per minute, although variations occur depending on metabolic demand, physical conditioning, and clinical status.

In clinical practice, calculating target cardiac output assists clinicians in evaluating circulatory adequacy, titrating vasoactive medications, assessing volume status, and determining thresholds for hemodynamic intervention. It is particularly useful in critical care settings, including shock, heart failure exacerbation, and perioperative monitoring. A target based on body surface area provides a personalized estimate of optimal cardiac performance, helping clinicians tailor therapy to each patient's physiologic needs.
